A current Emergency Treatment form is required for all junior girls competing in KWGA Junior events. The form need only be completed once during the 2008 season. If you have not yet completed the Emergency Treatment Form, click here prior to completing online registration. You may also print the form and submit the written registration form for entry into the Junior Season Opener.
